    Should the Baby Be Circumcised?

    ​At birth, most boys have skin that completely covers, or almost covers, the end of the penis. Circumcision removes some of this
    foreskin so that the tip of the penis (glans) and the opening of the urethra are exposed to air.

    Circumcision has been practiced as a religious rite for thousands of years. In the United States, the majority of male babies are circumcised for religious or social reasons.

    When is circumcision typically done?

    Routine circumcisions typically are performed in the hospital within a few days of birth. When done by an experienced physician, circumcision takes only a few minutes and complications are rare. After consultation with you, local anesthesia (numbing medicine) is provided during the procedure; the doctor should inform you in advance about the type of
    anesthesia they recommend.

    We recommend you talk with your obstetrician or pediatrician early in pregnancy about the pros and cons of circumcision. This can help you decide whether you want to have your baby circumcised.

    Potential risks & benefits of circumcision

    Studies have concluded that circumcised infants have a slightly lower risk of
    urinary tract infections during the first year after birth. Circumcision during the neonatal (newborn) period also provides some protection from penile cancer (a very rare condition, even in uncircumcised men).

    Some research also suggests a reduced likelihood of developing
    sexually transmitted diseases and
    HIV infections in circumcised men, and possibly a reduced risk for
    cervical cancer in female partners of circumcised men. However, while there are potential medical benefits, data are not sufficient to recommend routine neonatal circumcision.

    Circumcision does pose certain
    risks, such infection and
    bleeding. A small percentage of circumcised boys develop a condition called meatal stenosis, in which the urethral opening gets scarred or narrowed. This can cause deviation of the urinary stream as well as straining to urinate, or in extreme cases, urinary tract infection or inability to urinate. Some boys can develop scarring of the shaft skin to the head of the penis, called a skin bridge, which requires another procedure to fix.

    Although the evidence also is clear that infants experience pain with circumcision, there are several safe and effective ways to reduce the pain.

    When circumcision should not take place at birth

    If the baby is born
    prematurely, has an illness at birth, or has
    congenital abnormalities or blood problems, they should not be circumcised immediately. For example, if a condition called

    hypospadias
    is present, in which the infant’s urinary opening has not formed normally, your doctor will probably recommend that your baby boy
    not be circumcised at birth. In fact, circumcision should be performed only on stable, healthy infants.

    What About Omega-3s and Vegetarians’ Stroke Risk? 

    Does eating fish or taking fish oil supplements reduce stroke risk? 

    In my last video, we started to explore what might explain the higher stroke risk in vegetarians found in the EPIC-Oxford study. As you can see below and at 0:25 in my video Vegetarians and Stroke Risk Factors: Omega-3s?, vegetarians have a lower risk of heart disease and cardiovascular disease overall, but a higher risk of stroke. We looked into vitamin D levels as a potential mechanism, but that didn’t seem to be the reason. What about long-chain omega-3s, the fish fats like EPA and DHA? 

    Not surprisingly, their levels are found to be “markedly lower in vegetarians and particularly in vegans than in meat-eaters.” They’re about 30 percent lower in vegetarians and more than half as low in vegans, as you can see below and at 0:45 in my video

    According to “the most extensive systematic assessment of effects of omega-3 fats on cardiovascular health to date,” combining 28 randomized controlled trials, stroke has no benefit. There is evidence that taking fish oil “does not reduce heart disease, stroke or death,” or overall mortality, either. This may be because, on the one hand, the omega-3s may be helping, but the mercury in fish may be making things worse. “Balancing the benefits with the contaminant risks of fish consumption has represented a challenge for regulatory agencies and public health professionals.”  

    For example, dietary exposure to polychlorinated biphenyls (PCBs) may be associated with an increased risk of stroke. In one study, for instance, “neither fish nor intake of PCBs was related to stroke risk. However, with adjustment for fish intake,” that is, at the same fish intake, “dietary PCBs were associated with an increased risk of total stroke,” so the PCB pollutants may be masking the fish benefit. If we had a time machine and could go back before the Industrial Revolution and find fish in an unpolluted state, we might find that it is protective against stroke. Still, looking at the EPIC-Oxford study data, if fish were protective, then we might expect that the pescatarians (those who eat fish but no other meat) would have lower numbers of strokes since they would have the fish benefit without the risk from other meat. But, no. That isn’t the reality. So, it doesn’t seem to be the omega-3s either.

    Let’s take a closer look at what the vegetarians are eating.

    When it comes to plant-based diets for cardiovascular disease prevention, all plant foods are not created equal. There are two types of vegetarians—those who do it for their health, and those who do it for ethical reasons, like global warming or animals—and the latter tend to eat different diets. Health vegans tend to eat more fruits and fewer sweets, for instance, and you don’t tend to see them chomping down on vegan donuts, as shown below and at 2:41 in my video

    “Concerns about health and costs were primary motivations for [meat] reduction” in the United States. A middle-class American family is four times more likely to reduce meat for health reasons compared to environmental or animal welfare concerns, as you can see in the graph below and at 2:55 in my video

    But in the United Kingdom, where the EPIC-Oxford stroke study was done, ethics was the number one reason given for becoming vegetarian or vegan, as you can see in below and at 3:05 in my video.

    We know that “plant-based diets, diets that emphasize higher intakes of plant foods and lower intakes of animal foods, are associated with a lower risk of incident cardiovascular disease, cardiovascular disease mortality, and all-cause mortality”—a lower risk of dying from all causes put together—“in a general US adult population.” But, that’s only for healthy plant foods. Eating a lot of Wonder Bread, soda, and apple pie isn’t going to do you any favors. “For all types of plant-based diets, however, it is crucial that the choice of plant foods is given careful consideration.” We should choose whole fruits and whole grains over refined grains and avoid trans fats and added sugars. Could it be that the veggie Brits were just eating more chips? We’ll find out next. 

    Another strikeout trying to explain the increased risk. Could it be that the vegetarians were eating particularly unhealthy diets? Labels like vegetarian or vegan just tell me what is not being eaten. You can be vegetarian and consume a lot of unhealthy fare, like french fries, potato chips, and soda. That’s why, as a physician, I prefer the term whole food, plant-based nutrition. That tells me what you do eat. You eat vegetables and follow a diet centered around the healthiest foods out there.

    Underlying cardiovascular issues often have no symptoms, which is why heart disease goes undiagnosed in so many people until they have a heart attack or other major cardiac event.

    Getting regular checkups and knowing cardiac risk factors like blood pressure, cholesterol levels, and body weight for your age helps uncover red flags sooner, when lifestyle changes and medication can still make an impact. Catching early warning signs also allows closer monitoring.

    Diabetes

    Like heart disease risk, a diabetes diagnosis begins with knowing your blood sugar levels and if you have prediabetes. The disease can silently damage organs for up to 10 years before typical symptoms like blurred vision or frequent urination appear. Getting ahead of it early minimizes the likelihood of neuropathy, kidney disease, vision loss, and other severe complications. Lifestyle adjustments when blood sugar is only mildly elevated can potentially reverse the progression to diabetes altogether.

    Breast Cancer

    Thanks to improved awareness and screening techniques, doctors can now detect the vast majority of breast tumors at an early stage, when they are small and localized (stage 0 or 1) and often responsive to treatment.

    Monthly self-exams help women notice unusual lumps right away, while regular mammograms and clinical breast exams by specialists aid diagnosis for more subtle changes undetectable by touch. The earlier breast cancer treatment begins, the better the outcome.

    Colon Cancer

    Like breast cancer, when detected early, colon cancer has over a 90 percent 5-year survival rate. If the cancer spreads to distant organs and advances to stage 4, survival drops below 15 percent. Starting regular colon cancer screening methods like at-home tests or colonoscopies at age 45—sooner with certain risk factors—is key because there typically aren’t clear symptoms early on. Detecting and removing precancerous polyps prevents their progression to cancer.

    Mental Health Disorders

    Many mental health conditions have early signs and symptoms that can prompt earlier support and treatment. For example, statistically a majority of lifetime cases of anxiety, depression, bipolar disorder, and schizophrenia begin by the mid-20s.

    Paying attention to changes from your norm in sleep, mood, thinking, or behavior allows both you and your doctor to address issues sooner and reduce impacts with counseling, lifestyle adjustments, or medication. Especially for young people, early intervention can redirect life trajectories.

    The list goes on—monitoring prostate changes or skin for melanoma, doing oral self-checks to find mouth cancer early, getting regular eye exams to detect glaucoma progression before vision loss, tracking PAP smears to catch precancerous cervical cell changes right away. No matter your age or gender, prioritizing preventive visits gives clinicians a chance to assess disease risk and detect any abnormalities at the earliest, most treatable points.

    Prevention Through Lifestyle

    Of course, early detection is only part of the equation—true preventive health prioritizes reducing disease likelihood in the first place through positive lifestyle choices.

    As Dr. Flowers emphasizes, “We know from extensive research that certain daily habits and ways of living dramatically impact health trajectories over the long run. The most proactive thing anyone can do is make prevention through nurturing these healthy lifestyle fundamentals your number one priority.”

    What does a lifestyle focused on disease prevention look like on a daily basis? According to Dr. Flowers, it includes pillars like a balanced, nutrient-dense diet centered around whole foods like vegetables, fruits, whole grains, beans and lentils. It also means getting regular exercise, ideally 30 minutes a day at moderate intensity, 5 days a week.

    Remaining smoke-free or quitting smoking, limiting alcohol consumption, and maintaining a healthy body weight all help safeguard health. So does effectively managing stress through mindfulness practices like meditation or yoga. Quality sleep, both getting enough hours and not facing interruptions like sleep apnea, is also vital.

    And it’s critical to take any doctor-prescribed medications properly to treat existing conditions as a form of tertiary prevention.

    Dr. Flowers emphasizes that consistently building habits in these areas day after day, year after year is what moves the prevention needle. Then combining it with regular wellness visits, age-appropriate health screenings, and early detection testing as needed offers optimal prevention through both lifestyle and medical oversight.

    Study Says Even Light Smoking During Pregnancy Affects Newborn’s Health

    Smoking during pregnancy is known to raise the risk of preterm delivery, low birth weight, and restricted growth of the fetus. But can light smoking during pregnancy, just one or two cigarettes, still harm the newborn?

    A study published in Journal of Epidemiology and Community Health suggests “that there is no safe period and no safe level of cigarette smoking shortly before or during pregnancy.” The new study provides further evidence of the harmful effects of smoking during pregnancy, emphasizing the need to completely quit the habit for the baby’s health.

    The research team utilized data from the US National Vital Statistics System (NVSS) that has recorded a total of 15,379,982 live births between 2016 and 2019. After filtering out factors such as multiple births, high blood pressure or diabetes before pregnancy, and lack of information on the smoking status 3 months before and during their pregnancy, the researchers narrowed it down to 12,150,535 mother-infant pairs.

    The study noted that over 9% of them reported smoking cigarettes before pregnancy, 7% during the first trimester, 6% in the second trimester, and around 6% in the third trimester. The intensity of smoking was measured based on the number of cigarettes smoked per day: 0, 1–2, 3–5, 6–9, 10–19, and 20 or more.

    “The women who smoked had more risk factors for neonatal health issues: they tended to be younger, non-Hispanic White, unmarried, and obese, and to have low educational attainment, more previous births, and fewer attendances for prenatal care,” the news release stated.

    The estimated prevalence of neonatal health issues was around 9.5%. Neonatal health issues included cases where newborns required immediate assisted ventilation, ventilation for more than 6 hours, NICU admission for continuous mechanical ventilation, surfactant replacement therapy, as well as cases of suspected sepsis, seizures, or serious neurological problems.

    “Smoking either before pregnancy or in each of the 3 trimesters of pregnancy was associated with an increased risk of major neonatal health issues, either separately or combined, after adjusting for potentially influential factors, including age, ethnicity, and weight (BMI) before pregnancy,” the news release stated.

    In cases where expectant mothers smoked before pregnancy, there was more 27% higher risk of having more than one major neonatal health issue. The risk was 31-32% higher if she smoked at any time during her pregnancy.

    When compared to expectant mothers who did not smoke at all, both smoking before pregnancy and during any point of pregnancy puts newborns at higher risk of having more than one health issue.

    “Even light smoking—1–2 cigarettes a day—was associated with a heightened risk of major neonatal health issues. For example, among mothers who smoked 1–2 cigarettes a day before pregnancy, the risk was 16% higher, rising to 31% higher if they smoked 20 or more a day,” the news release stated.

    When an expectant mother smokes just one or two cigarettes a day, the newborn’s risk of admission to intensive care is 13% higher. This risk increases to 29% for those who smoke 20 or more cigarettes daily.
